Street outreach and mobile medicine serve unsheltered clients

January - June 2024: By the numbers

In partnership with SALA medics, our street outreach teams provide access to medical supplies and care to unsheltered clients reached during pop-up events and visits to camps. Clients who are identified with high medical needs during outreach are added to our Highly Vulnerable Adult Case Conferencing and, when appropriate, referred to the Complex Care facility.

Housing is a fundamental social determinant of health, influencing both physical and mental well-being. Experiencing homelessness exacerbates health issues, leading to higher rates of chronic illnesses, mental health conditions, and premature mortality.

By bringing health care directly to unsheltered clients, street medicine bridges the gap in access, providing essential medical services, building trust and ultimately improving health outcomes for some of the most vulnerable members of our community.
